Shift Work, Chronotype, and Melatonin Patterns among Female Hospital Employees on Day and Night Shifts.

چکیده
BACKGROUND Shift work-related carcinogenesis is hypothesized to be mediated by melatonin; however, few studies have considered the potential effect modification of this underlying pathway by chronotype or specific aspects of shift work such as the number of consecutive nights in a rotation. In this study, we examined melatonin patterns in relation to shift status, stratified by chronotype and number of consecutive night shifts, and cumulative lifetime exposure to shift work. METHODS Melatonin patterns of 261 female personnel (147 fixed-day and 114 on rotations, including nights) at Kingston General Hospital were analyzed using cosinor analysis. Urine samples were collected from all voids over a 48-hour specimen collection period for measurement of 6-sulfatoxymelatonin concentrations using the Buhlmann ELISA Kit. Chronotypes were assessed using mid-sleep time (MSF) derived from the Munich Chronotype Questionnaire (MCTQ). Sociodemographic, health, and occupational information were collected by questionnaire. RESULTS Rotational shift nurses working nights had a lower mesor and an earlier time of peak melatonin production compared to day-only workers. More pronounced differences in mesor and acrophase were seen among later chronotypes, and shift workers working ≥3 consecutive nights. Among nurses, cumulative shift work was associated with a reduction in mesor. CONCLUSION These results suggest that evening-types and/or shift workers working ≥3 consecutive nights are more susceptible to adverse light-at-night effects, whereas long-term shift work may also chronically reduce melatonin levels. IMPACT Cumulative and current exposure to shift work, including nights, affects level and timing of melatonin production, which may be related to carcinogenesis and cancer risk. Investigating the effect of night shifts rotation speed on the pattern of melatonin secretion and sleepiness among petrochemical control room operators
Introduction: This paper aims to explore the main effect of night shift rotation speed on the pattern of melatonin secretion and the sleepiness, among control room operators (CORs) in a petrochemical industry Material and method: In this study, 60 CORs operators within two different patterns, including 7 nights and 3 nights shift works were selected from a petrochemical industry to investigate...
متن کاملNursing Time Allocation: A Wok Sampling Survey in a Turkish Private Hospital
Background and Objectives: Work sampling is a useful technique to characterize how employees allocate their work time to various activities. Given the value of nurses’ time for quality health care delivery, characterization of nursing time distribution may provide useful information for optimal use of nursing staff. The purpose of the present study was to explore time allocatio...

Background and Objective: According to previous studies shift work could desynchronize the natural circadian rhythm of the body. Although some of the internal physiologic processes become active for adaptation of the body with this desynchronization. One of these physiologic processes is endocrine system and melatonin release. This hormone is one of the most important variants which represent t...
متن کاملSleep Strategies of Night-Shift Nurses on Days Off: Which Ones are Most Adaptive?
OBJECTIVES To determine the off-shift sleep strategies of bi-ethnic night-shift nurses, the relationship between these sleep strategies and adaptation to shift work, and identify the participant-level characteristics associated with a given sleep strategy. METHODS African-American and non-Hispanic White female, night-shift nurses from an academic hospital were recruited to complete a survey o...
متن کاملLight exposure and melatonin among rotating shift nurses--letter.
Grundy and colleagues (1) conducted a study into how light influencesmelatonin in nurses during rotating shifts. Inter alia, it was concluded that the examined "two-12hour-days/two-12-hour-nights/5-days-off" pattern minimally disrupts melatonin production. Interestingly, chronotype information was obtained but appeared to have negligible effects on shift-work–melatonin relationships. The author...
